Id leave out the personal details, unless your referring to name and contact information. Include things like your age, gender, race, marital/dependents status, and nationality for your cover letter.

If your with a service forget the references entirely. Include a reference list with your reference scans when requested.

Recruiters and ISs care about 2 things: What you have taught and what you can teach. This means education (training) certifications, teaching (educator) experience and special skills (skills that could fill an extra curricular need of the school).
