This might be a ridiculous question as one of the key words is confidential, but is the Confidential Supervisor Evaluation Form available anywhere online? An internet search came up with a copy a teacher had posted of one of her forms, but it's from 2008.

I'm keen to have a peek as to how extensive the form is or how much writing is involved as it will have a direct impact on who I ask to act as references.

Its a series of about 10 questions asking to rate you 1-5, with a box for typing in any further comments by your reference.

10 questions, 1 further comment box and then a list of 15 characteristics to be rated from A to F.

Thanks for the information. It will certainly help me make the decision as to which of my references will be most likely to complete the forms promptly at this busy time of year.
