Has anyone had to ask for a letter of professional standing to "convert" a US licence to the UK QTS equivalent? Who did you contact?
I emailed the Office of the State Superintendent of Education (I have a DC licence) but I haven't gotten any response.

You are not converting the credential youre applying for mutual recognition to gain an additional credential, you get to keep your DC credential once QTS is granted.
Write a letter for an official statement of verification and include the following in the letter:
1) First and Last Name
2) Mailing address
3) Social Security Number
4) Date of Birth
5) The subject areas you want in the letter
Send the request too:
Office of the State Superintendent
Dept. of Elementary and Secondary Education
Office of Educator Licensure & Accreditation
810 First Street NE, 5th Floor
Washington, DC. 20002

There is no fee for the request and processing time is about 2 weeks.
There are two fields available in your TCL profile for subject areas, in your letter list the subjects you have a credential in for those two areas.
